When a proposed use of any
copyrighted work appears to go beyond fair use limitations, the Libraries
Copyright Office will seek the required clearances for educational use, and pay
any applicable royalty fees. Please
provide all citation information.
These are procedures for all instructors submitting media for Library Media Reserves for student use. In addition to a Library Course Reserves form signed by you (for contact information and course identification) for each course,
PLEASE PROVIDE:
1. A list of titles and call numbers for Library owned media.
2. Typed citations for all personally owned media to be reserved for each course.

In whatever bibliography format you use, each citation must include:
i. Director
ii. Title
iii. Media format (CD, DVD etc.)
iv. Producer and Distributor
v. Year of release
b. Any written permission letters you have received from colleagues, or any other preauthorization to use copyrighted materials, such as student permission forms.
c. Library staff will verify citations and find missing information, if an instructor is unable to provide complete bibliographic information.
4. If you have taped a television program (“off-air taping”), this is a fair use for the classroom and for Library Reserves, for a limited time. Be sure to label the tape with the date and time of the broadcast, and the title information.
