Signature Sheets for all university schools can be found below. They are all different, so it is extremely important that you make sure that you are using the correct one.
Please note the following EXTREMELY IMPORTANT information regarding your Signature Sheet:
- PLEASE BE SURE THAT YOU ARE USING THE CORRECT SIGNATURE SHEET FOR YOUR COLLEGE/SCHOOL/DEGREE.
- If your degree/discipline's Signature Sheet template does not contain names (either real names of real persons or fake names such as "Dr. Firstname Lastname" or "Dr. Asdf Ghjkl) under "Committee:":
- Do not insert your committee members' names when you create your own Signature Sheet.
- If no real or fake names are included, and there is only a title (e.g., "Chair," "Program Director") next to a signature line, do not supply that person's name.
- If there is a blank space (i.e., there is neither a name nor a title) next to a signature line under "Committee:", leave the space blank.
